The consultation appointment is important because it allows us to understand your oral surgery goals. Then, we’ll design a plan on how to achieve them. You can complete the registration form before the appointment, which will save some time on the day of your consultation.

What to Bring

We want to make the most of every appointment. Please arrive a few minutes early to your consultation along with some items:

  • Your health and/or dental insurance information
  • A list of current or prior medical conditions, including diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.
  • A list of current medications
  • Referral forms and/or X-rays, if any
  • A valid driver’s license or photo ID
  • Form of payment

Parents of Minors

Patients under the age of 18 must be accompanied to the consultation by a parent or legal guardian.

What to Expect

Your first visit with our board-certified surgeons is a great time for us to get to know each other. We design a treatment plan for every patient based on their condition, preference, and goals for improved oral health.

A typical consultation visit includes a medical review, a comprehensive oral exam, 3D imaging, and a discussion of your anesthesia options. This first appointment is also a great time to ask questions.

Financial & Insurance Information

We make every effort to ensure our oral surgery services are as accessible to our patients as possible. We accept several options for payment and are happy to submit a claim to your insurance on your behalf, provided we have all of the necessary information.

Payment is due at the time of service unless prior arrangements have been made. We are happy to discuss payment arrangements, but this must be done prior to your treatment.
